Just found two guides, which between them have saved our SharePoint farm from the scrapheap.

A great resource – detailed, and very nearly worked. However, because we were not starting with a fresh install, as it suggests, I am guessing there were some remnants left over from our previous attempts at making this work. In short, the User Profile Synchronization Service was showing as Starting, even with no User Profile Application service set up.
To stop this ‘fake’ service from showing as starting, and allow the creation of the new User Profile Application to configure everything properly, Kathy Hughes provided information on using PS to identity and force the stop of the service. Once both User Profile Service & User Profile Synchronization are shown as stopped, then continue with original information.
User Profile Sync now working, and helped a lot of other parts too. Really pleasing. Thanks to the above two posts, everything is working.
